  • You can go to a VITA site to get your tax forms filled out for FREE.
    Click here for an updated list of 2005 VITA sites and schedules.

    When you go to a VITA site, take along:

    • ALL your W-2s
    • ALL your other income information (like Forms 1099 if you get those)
    • ALL the Social Security Numbers for your family
           (take the actual Social Security cards if you can)
    • Picture identification
    • A copy of last year’s tax return (if you can find it)
    • A check or deposit slip for your bank account (if you want direct deposit of your refund)
    • If you own a home, your property tax receipt from the City and your interest statement from your bank (Form 1098)
    • If you make estimated tax payments, records of that (like cancelled checks)
    • If you want the Wisconsin Homestead Credit, a completed rent certificate, and a printout of any W-2 payments you got during the year
