How do you apply to get health insurance (in Milwaukee County) ?

  1. Decide how you want to apply:
    1. Call Application Registration at 287-7477,
      OR
    2. Go to an application office and pick up an application form (good idea to call first),
      OR
    3. Call to schedule a face-to-face appointment at an application office.
       
      4. !!! If you are elderly or disabled, you must have a face-to-face appointment.

  2. You will need:
    1. Proof of your and your family’s Social Security Numbers.
    2. Proof of citizenship or alien status.
    3. If you're pregnant, a signed and dated note from your doctor saying what your due date is.
       
      4. !!! If you are elderly or disabled, call first to see what other papers you need to take to your appointment.
